The appointment request form on this site collects only the following:

The form is not designed to collect information about your medical condition and must not be used for that purpose. Health data is classified as special-category personal data under KVKK; assessment takes place only during an in-person examination. Please do not enter details about your medical condition into the free-text field.

Your data is processed solely to forward your appointment request and to allow us to respond, on the basis of your explicit consent. It is not used for any other purpose and is never used for marketing messages.

Form content is not stored in any database. Your request is delivered by email directly to the physician’s own mailbox at the moment you submit it, and no record is kept in the site infrastructure.

The site is hosted on Cloudflare infrastructure and an email delivery service is used to forward form submissions. Servers of these providers may be located outside Türkiye, which means your data may be transferred abroad when you submit the form. By submitting the form you give explicit consent to this transfer.

Under Article 11 of KVKK you have the right to learn whether your personal data is processed, to request information about such processing, to learn its purpose, to know the third parties to whom it is transferred domestically or abroad, to request correction of incomplete or inaccurate data, to request erasure or destruction, and to object to processing.
